Traveller’s diarrhoea – what exactly is it?
For many of us, holidays are a time for relaxation, sunshine and new flavours. Unfortunately, they can just as often pose a challenge for our digestive system. One of the most common problems that can ruin even the best-planned holiday is traveller’s diarrhoea.
It is an acute gastrointestinal infection that usually affects people travelling to countries with lower standards of hygiene. Symptoms include frequent, loose stools, abdominal pain, nausea and sometimes a fever. Most cases are mild, but it can effectively ground a traveller for a few days – and put a stop to their holiday plans.
Where is the risk greatest?
The risk is not the same everywhere. According to WHO data, traveller’s diarrhoea most commonly affects people visiting:
- Southeast Asia (India, Thailand, Indonesia),
- Africa (particularly North Africa and Sub-Saharan Africa),
- Latin America (Mexico, Peru, Brazil),
- and the Middle East.
The risk is moderate in Eastern Europe and the Balkans, and lowest in Western Europe, the USA, Canada, Australia and Japan.
What causes traveller’s diarrhoea?
The main cause is bacteria – they account for as many as 80% of cases. The most common culprit is enterotoxigenic Escherichia coli (ETEC), but Shigella, Salmonella and Campylobacter are also frequent causes, as are viruses (e.g. noroviruses) and parasites (e.g. Giardia lamblia).
Infection occurs through the consumption of contaminated water, ice, raw fruit and vegetables, undercooked meat or unpasteurised milk. Sometimes, just a single sip of local water or an ice cube in a drink is enough.
How can you protect yourself against diarrhoea whilst travelling?
Prevention is key. Good hygiene, taking care when eating and drinking, and boosting gut immunity can significantly reduce the risk.
Rules worth following:
- Drink only bottled or boiled water – even when brushing your teeth.
- Avoid ice in your drinks (it is often made from tap water).
- Eat only well-cooked, fresh meals – preferably served hot.
- Peel your own fruit and vegetables, or eat those that can be blanched.
- Wash your hands before meals or use hand sanitiser.
- Avoid raw seafood, unpasteurised cheese and street food of unknown origin.
It’s also a good idea to boost your gut flora with a suitable probiotic before you travel – and take some with you just in case.
